NPI
Overview
The Health Insurance Portability and Accountability Act (HIPAA) of 1996 requires the adoption of a standard unique identifier for health care providers, the National Provider Identifier (NPI). The NPI final rule issued January 23, 2004 adopted the NPI as this standard. The NPI is 10 digits in length and will replace health care provider identifiers in use today, including the nine-digit Medi-Cal provider number. To comply with this requirement, the Centers for Medicare & Medicaid Services (CMS) developed the National Plan and Provider Enumeration System (NPPES) to assign these unique identifiers.
NPPES began accepting applications and issuing NPIs, on May 23, 2005. All health care providers (e.g., physicians, suppliers, hospitals, etc.) are encouraged to obtain their NPI as quickly as possible. A hard copy application may be requested by contacting the NPPES contractor at 1-800-465-3203 (NPI toll-free). Applications can be submitted online at the NPPES Web site or by mail to:

When applying for an NPI, CMS urges providers to include their legacy identifiers for all payers (e.g., health insurance plans, State Medicaid agencies, Medicare). If reporting a Medicaid number, include the associated state name. This information is critical for payers in the development of crosswalks to aid in the transition to the NPI.
Covered health care providers must obtain an NPI by May 23, 2007. For additional questions regarding NPI, contact the Telephone Service Center (TSC) at 1-800-541-5555, select language preference (option 11 for English; option 12 for Spanish), select option 16 from the main menu, then select option 18 from the sub menu.
Complete Implementation of NPIs Updated September 3, 2008 Medi-Cal and the Child Health and Disability Prevention (CHDP) program currently accept the National Provider Identifier (NPI) on all paper and electronic claims and transactions. To comply with the federally mandated NPI utilization date of May 23, 2008, providers should use their NPI instead of the legacy provider number. Providers that do not use an NPI on Medi-Cal and CHDP claims are out of compliance. Medi-Cal and CHDP will continue to accept legacy provider numbers through September 30, 2008 only. Providers that have registered their NPI but have not submitted claims using their NPI will not experience interruption in payments for claims submitted with a legacy provider number. However, effective October 1, 2008, claims submitted with a legacy provider number will be denied. The exceptions to this requirement for Medi-Cal claims are blood banks, Christian Science Practitioners and Multipurpose Senior Services Program providers. These providers may continue to submit Medi-Cal claims with a legacy provider number. The exception is also applicable to Medi-Cal Managed Care plans when reporting CHDP health assessment information on the CHDP Assessment Confidential Screening/Billing Report (PM 160INF) form. The Department of Health Care Services (DHCS) will continue to end-date all Medi-Cal and CHDP legacy provider numbers for those providers who successfully submit a claim using an NPI. Secondary Provider IdentifiersIn accordance with the NPI final rule, all provider identifiers, including secondary providers, must be an NPI. For the secondary provider identifier fields, the billing provider should attempt to obtain the NPI for use on the claim by either contacting the provider or obtaining the NPI information through the NPI Registry. For Medi-Cal claims, below are the instructions for secondary provider identifier use if all attempts to acquire the NPI are unsuccessful:
Medi-Cal will allow the use of the Medi-Cal provider number or license number in the fields identified above while providers transition to sole use of the NPI. To comply with the NPI final rule, Medi-Cal plans to accept only the NPI in secondary provider fields. The implementation of NPI utilization for secondary providers will be published in a future Medi-Cal Update. Crossover ClaimsIf a Medicare claim crosses over electronically to Medi-Cal for payment, the NPI used to bill Medicare must be registered with Medi-Cal or the claim will not be processed. If Medicare rejects a provider’s claims for not using the NPI, the provider must resubmit the claim to Medicare. Medicare must process the claim before Medi-Cal can pay the crossover claim. Resources will continue to be available to assist in the registration of the NPI through Medi-Cal’s Telephone Service Center (TSC) at 1-800-541-5555.
